OR in person on July 19, 2025 at 8:00 AM and 1:00 PM at the BukSU Auditorium.

• Soft copy of a 2×2 ID picture

• Scanned copy of Transcript of Records (TOR)

• Scanned copy of Honorable Dismissal

• Proof of payment of admission exam fee (₱675.00)
