3️⃣ Certified True Copy of Transcript of Records (TOR)
4️⃣ Certification of GWA (80% and above)
Issued by the Registrar and noted by the Dean of the school where the applicant graduated.
5️⃣ Certified True Copy of Diploma
6️⃣ NMAT Result (Photocopy)
Minimum required score: 60
7️⃣ Honorable Dismissal from the last school attended
(Not required for BukSU graduates)
 
𝐈𝐧𝐭𝐞𝐫𝐞𝐬𝐭𝐞𝐝 𝐚𝐩𝐩𝐥𝐢𝐜𝐚𝐧𝐭𝐬 𝐭𝐨 𝐭𝐡𝐞 𝐁𝐮𝐤𝐒𝐔 𝐂𝐎𝐋𝐋𝐄𝐆𝐄 𝐎𝐅 𝐌𝐄𝐃𝐈𝐂𝐈𝐍𝐄 𝐚𝐫𝐞 𝐚𝐝𝐯𝐢𝐬𝐞𝐝 𝐭𝐨 𝐫𝐞𝐯𝐢𝐞𝐰 𝐭𝐡𝐞 𝐟𝐨𝐥𝐥𝐨𝐰𝐢𝐧𝐠 𝐫𝐞𝐪𝐮𝐢𝐫𝐞𝐝 𝐜𝐫𝐢𝐭𝐞𝐫𝐢𝐚:
 
1️⃣ Must be a 𝗙𝗶𝗹𝗶𝗽𝗶𝗻𝗼 citizen
 
2️⃣ Must hold at least a baccalaureate degree, preferably in the health sciences.
The degree must have been completed within 5 years from the time of application.
 
3️⃣ If the degree was completed more than 5 years prior to application, the applicant is required to take a health-related refresher course for at least one semester totaling 12 units before applying to the College of Medicine.
 
Recommended refresher subjects:
 
Biochemistry
Comparative Vertebrate Anatomy
Organic Chemistry
 
Exemption: This refresher requirement may be waived if the applicant has been involved in health-related research work or has worked in a healthcare institution.
 
4️⃣ Preferably a member of the Indigenous Cultural Community (ICC) and coming from Geographically Isolated and Disadvantaged Areas (GIDA), or conflict- or disaster-prone areas in the Philippines.
 
5️⃣ Must have a National Medical Admission Test (NMAT) result of at least 60, taken within 2 years prior to admission.
 
6️⃣ Must have a General Weighted Average (GWA) of at least 80%.
 
7️⃣ Must be of good moral character and must not have been convicted with finality of a crime involving moral turpitude.
